New students will need a copy of a Birth Certificate, Social Security Card and current immunization records. A health assessment is required for new students enrolling in a Kansas school for the first time. Records from your previous school will be requested by USD 506 staff to be sent directly to your school.
A sports physical is required if the student is participating in sports. The sports physical form can be found (HERE).
